A photocopy of your travel health insurance policy needs to be sent to me as soon as you have completed your travel Insurance policy form.

Be sure to check if your personal health insurance policy covers travel overseas including the countries of Moldova, and/or Ukraine.

For entrance to Ukraine, verification of travel insurance is required. Also, by signing below, you are consenting to receiving emergency medical treatment which may be deemed advisable by tour officials in the event of injury, accident, or illness and for which you may be unable to give your consent to the time, due to incapacity as a result of the injury, accident, or illness.
